PERITONSILLAR ABSCESS -ETIOLOGY,MECHANISM,CLINICAL FEATURES,TREATMENT

 PERITONSILLAR ABSCESS ( QUINSY)


1)INTRODUCTION:

TONSIL-Subepithelial aggregation of lymphoid tissue which forms a part of waldeyer's ring

  • Its ovoid in shape
  • Situated in lateral wall of oropharynx

QUINSY- Collection of pus in peritonsillar space between capsule and superior constrictor muscle


2)ETIOLOGY:

  • As a sequelae of acute tonsillitis
  • De novo 
Causative organism : 

  • Streptococcus pyogenes
  • Staphylococcus aureus 
  • Anaerobic organism  





3)MECHANISM :

One of the crypts or crypta magna gets filled 

With pus and gets sealed off

Intratonsillar abscess 

Burst through capsule

Peritonsillitis

Peritonsillar abscess 


4)CLINICAL FEATURES :

GENERAL;

  • High grade fever with chills
  • Malaise
  • Headache
  • Body ache
  • Nausea

LOCAL:

  • Foul breath
  • Trismus
  • Severe unilateral throat pain
  • Poor orodental hygiene
  • Hot potato speech


5)EXAMINATION:

  • UVULA:Swollen ,pushed to opposite side
  • Tonsil,soft palate :congested on involved side
  • Torticollis:head tilted to involved side
  • Mucopus covering the tonsil 





6)TREATMENT:

CONSERVATIVE :

  • Hospitalisation and IV fluids
  • Analgesic 
  • Oral hygiene : Warm saline saline gargle

SURGICAL ;

a)INCISION AND DRAINAGE: 

  • If frank ps is present incision is done 
  • Incision point : Point of maximum bulge 

b)INTERVAL TONSILLECTOMY:

  • 4-6 Weeks following quincy 
  • Abscess tonsillectomy:
  • Disadvantage: 
  • Increased bleeding 






7)COMPLICATION :

  • Septicaemia
  • Endocarditis
  • Laryngeal oedema
  • Pneumonitis
  • Parapharyngeal abscess
